Portage County Health Department Vaccination Clinic for JR/SR High Students

Two vaccinations are required for incoming 7th and 12th graders for the 2017-2018 school year. The state of Ohio requires all students entering the 7th grade to have the meningococcal vaccine (also known as MCV4, Menevo or Menactra) in addition to one dose of Tdap (Tetanus, diphtheria and pertussis).  The state of Ohio also requires all students entering the 12th grade to have a second dose of meningococcal vaccine (also known as MCV4, Menevo or Menactra).   If the first dose of meningococcal vaccine was administered on or after the 16th birthday, a second dose is not required. To help students and parents, these vaccinations will be available at a clinic provided by the Portage County Health Department scheduled for May 8th at the JR/SR High from 12:00-2:00.  Any student who needs caught up with vaccinations can participate in this clinic with the proper documentation; however, these two vaccinations are being highlighted since they are an absolute requirement for 7th and 12th graders. Please fill out the permission form that was sent home and send it back with your student before May 8th. Students who do not provide documentation of these immunizations to the school by the Fall of 2017 are subject to exclusion.  If you need additional forms, click below.
